This may include your name, contact details, medical history, dietary habits, test results, and current medications. How Your Information Is Used Your information is used for the following purposes: Assessing suitability for nutrition services Developing and updating treatment plans Referrals for functional/pathology testing (e.g., microbiome, SIBO) Communication with your GP or other healthcare providers (with your consent) Appointment reminders, billing, and administrative functions Legal or regulatory requirements (e.g., subpoenas) Your data will not be disclosed without your written consent unless required by law. All digital records are stored securely. Fees & Cancellation Policy Fees Fees are payable at the time of booking consultation. We accept Visa, Mastercard, EFTPOS, and bank transfers. (Surcharges may apply.) Cancellations To help us serve all clients effectively: 24+ hours' notice is required for rescheduling or cancellations. Cancellations with less than 24 hours’ notice will incur a 50% cancellation fee. No-shows will be charged the full consultation fee. If you are unwell or experiencing extenuating circumstances, please notify us as early as possible to discuss options. Informed Consent By commencing care with ClinicalNutritionbyOla, you confirm that: You understand that nutritional consultations are not a substitute for medical diagnosis or treatment. The service provided is focused on dietary, lifestyle, and nutritional support to promote general health and wellbeing. You will inform your practitioner of any changes to your medications, health status, or relevant test results. You will not cease or alter prescribed medication without guidance from your medical practitioner.
