Why Thyroid Health Deserves Attention During the Menopausal Transition
Many women entering their 40s begin to notice changes in their health.
Fatigue that doesn't improve with rest, brain fog, weight gain despite eating well, hair thinning, dry skin and heavy or irregular periods are often dismissed as "just perimenopause."
While these symptoms are common during the menopausal transition, they are not always caused by changing reproductive hormones alone.
For some women, thyroid dysfunction—particularly autoimmune thyroid disease such as Hashimoto's thyroiditis—may also be contributing to how they feel.
The challenge is that the symptoms of perimenopause, thyroid dysfunction and even insulin resistance often overlap, making it difficult to recognise what is really happening.
Some integrative medicine practitioners use the term "thyropause" to describe this overlap between perimenopause and hypothyroidism. Although thyropause is not an official medical diagnosis, it is a useful concept that highlights the close relationship between hormonal changes, immune function and thyroid health during midlife.
Rather than asking:
"Is it menopause or my thyroid?"
A better question may be:
"Could more than one factor be contributing to my symptoms?"

What is Thyropause?
Thyropause is a descriptive term used by some healthcare practitioners to explain the overlap between:
perimenopause
thyroid dysfunction
autoimmune thyroid disease
It recognises that many women first develop thyroid symptoms during the menopausal transition.
This does not mean that perimenopause directly causes thyroid disease. Instead, hormonal changes, ageing, genetics, immune function and lifestyle factors may all interact during this stage of life.
Looking at the whole clinical picture can help guide appropriate medical assessment and personalised nutrition support.
Understanding Perimenopause
Perimenopause is the transitional stage leading up to menopause.
It commonly begins during a woman's 40s but may start earlier or later.
During this time the ovaries gradually change hormone production.
Rather than declining steadily, oestrogen levels often fluctuate significantly while progesterone gradually decreases.
These hormonal changes affect far more than the menstrual cycle.
Oestrogen and progesterone influence:
brain function
metabolism
bone health
cardiovascular health
immune regulation
sleep
mood
muscle function
thyroid hormone transport
Because these hormones interact with many body systems, symptoms can vary greatly between women.
Why Does Thyroid Disease Become More Common During Midlife?
Women are around five to eight times more likely than men to develop thyroid disease.
Hashimoto's thyroiditis is the most common cause of hypothyroidism in Australia and other Western countries.
Researchers believe several factors contribute to the increased risk during midlife.
1. Hormonal Changes
Sex hormones influence immune function and thyroid hormone regulation.
During perimenopause, fluctuating oestrogen and declining progesterone may alter the way the body responds to thyroid hormones.
Women already taking thyroid medication may notice changes in their thyroid requirements during this stage of life.
Regular monitoring with a GP is important.
2. Immune System Changes
The immune system naturally changes with age.
Current research suggests hormonal transitions may influence immune regulation in genetically susceptible individuals.
Hashimoto's thyroiditis develops when the immune system mistakenly attacks thyroid tissue, gradually reducing thyroid hormone production.
Not every woman experiencing perimenopause will develop autoimmune thyroid disease, but the incidence increases during midlife.
3. Insulin Resistance
Insulin resistance also becomes more common during perimenopause.
Changing hormones, reduced muscle mass, altered body composition and lifestyle factors may all contribute.
Interestingly, many symptoms overlap with both thyroid dysfunction and perimenopause.
Why Are Symptoms So Similar?
One reason thyroid disease is frequently overlooked during perimenopause is because the symptoms are remarkably similar.
Common overlapping symptoms include:
Fatigue
Brain fog
Weight gain
Hair thinning or hair loss
Heavy or irregular periods
Elevated cholesterol
Constipation
Feeling cold
Dry skin
Muscle aches
Poor concentration
Reduced exercise tolerance
These symptoms are not specific to one condition.
They may be influenced by:

hypothyroidism
insulin resistance
iron deficiency
vitamin B12 deficiency
vitamin D deficiency
poor sleep
chronic stress
other medical conditions
For this reason, persistent symptoms should not automatically be attributed to menopause alone.
The Connection Between Perimenopause, Thyroid Disease and Insulin Resistance
One of the most helpful ways to understand thyropause is to view these conditions as overlapping circles rather than isolated problems.
Many women experience more than one contributing factor simultaneously.
For example:
Perimenopause may influence metabolism and body composition.
Insulin resistance may contribute to weight gain and fatigue.
Thyroid dysfunction may affect energy levels, cholesterol and hair health.
Rather than asking which condition is responsible, healthcare practitioners aim to identify which factors may be contributing to an individual's symptoms.
Could It Be Hashimoto's Thyroiditis?
Hashimoto's thyroiditis is an autoimmune condition in which the immune system gradually damages thyroid tissue.
Over time this may reduce thyroid hormone production and lead to hypothyroidism.
Hashimoto's often develops slowly.
In many people, thyroid antibodies are present years before thyroid hormone levels become abnormal.
Symptoms may include:
persistent fatigue
brain fog
dry skin
constipation
feeling cold
hair thinning
weight changes
heavy menstrual bleeding
elevated cholesterol
These symptoms overlap considerably with perimenopause.
What Tests Might Your GP Consider?
If thyroid dysfunction is suspected, your GP may consider appropriate investigations based on your symptoms and medical history.
These may include:
Thyroid function
TSH
Free T4
Free T3 (when clinically indicated)
Autoimmune thyroid disease
Thyroid peroxidase antibodies (TPOAb)
Thyroglobulin antibodies (TgAb)
Depending on your presentation, your GP may also consider:
Iron studies (including ferritin)
Vitamin B12
Folate
Vitamin D
Lipid profile
HbA1c or other investigations where clinically appropriate
The most suitable investigations vary between individuals.
Understanding TSH
TSH (thyroid-stimulating hormone) is produced by the pituitary gland.
When thyroid hormone levels fall, the pituitary produces more TSH to stimulate the thyroid.
Generally:
Higher TSH may suggest reduced thyroid function.
Lower TSH may suggest increased thyroid activity.
However, thyroid assessment is never based on TSH alone.
Healthcare professionals also consider:
Free T4
symptoms
thyroid antibodies
medical history
previous blood tests
medications
pregnancy status where relevant
Can Thyroid Medication Requirements Change During Perimenopause?
For women already taking levothyroxine, the answer is sometimes yes.
One reason relates to a protein called thyroxine-binding globulin (TBG).
Higher oestrogen levels increase TBG.
This protein binds thyroid hormone in the bloodstream, leaving less available in its free form.
For some women, this may influence thyroid medication requirements, particularly when starting oral menopausal hormone therapy.
Regular thyroid function testing allows medication doses to be adjusted when necessary.
Medication changes should always be guided by the prescribing doctor.
Why Do Some People Continue to Experience Symptoms?
Levothyroxine (T4) is the recommended first-line treatment for hypothyroidism and is highly effective for most people.
However, some individuals continue to experience symptoms despite blood tests within the target range.
Persistent symptoms do not necessarily mean thyroid medication is ineffective.
Other contributing factors may include:
iron deficiency
vitamin B12 deficiency
vitamin D deficiency
sleep disorders
stress
insulin resistance
perimenopause
other medical conditions
A comprehensive assessment helps identify potential contributing factors rather than assuming every symptom is thyroid related.
The Gut–Thyroid Axis: An Emerging Area of Research
Research continues to explore the relationship between the gut microbiome, immune system and thyroid health.
Scientists are investigating how the gut microbiome may influence:
immune regulation
intestinal barrier function
inflammation
nutrient absorption
thyroid hormone metabolism
Although research is promising, nutrition should not be viewed as a treatment for autoimmune thyroid disease.
Supporting gut health through a varied, fibre-rich diet may contribute to overall wellbeing and digestive health.
Gluten and Hashimoto's
Gluten is one of the most discussed topics in thyroid nutrition.
There is strong evidence that people with Hashimoto's have a higher prevalence of coeliac disease than the general population.
If coeliac disease is suspected, appropriate testing should be discussed with your GP before removing gluten from your diet.
For individuals without coeliac disease, current research is mixed.
Nutrition recommendations should always be individualised based on symptoms, dietary intake, medical history and appropriate investigations.
Selenium and Thyroid Health
Selenium is an essential trace mineral involved in:
normal thyroid hormone metabolism
antioxidant defence
immune function
Some clinical studies have investigated selenium supplementation in autoimmune thyroid disease, with mixed findings.
Routine supplementation is not appropriate for everyone.
If supplementation is being considered, it should be discussed with your healthcare practitioner because excessive selenium intake can be harmful.
What About Iodine?
Iodine is essential for the production of thyroid hormones.
However, more is not always better.
In Australia, iodine deficiency is less common due to food fortification.
For people with autoimmune thyroid disease, excessive iodine intake may worsen thyroid autoimmunity in susceptible individuals.
For this reason, iodine supplements should only be used under medical supervision unless specifically recommended by your doctor.
Supporting Your Health During Perimenopause
Although no specific diet has been proven to prevent or cure thyroid disease, healthy lifestyle habits can support overall wellbeing.
Practical strategies include:
Eating adequate protein at each meal to support muscle health and healthy ageing.
Including plenty of colourful vegetables, fruit, legumes and wholegrains (unless medically advised otherwise).
Consuming healthy fats from foods such as extra virgin olive oil, nuts, seeds and oily fish.
Supporting gut health with a variety of fibre-rich plant foods and fermented foods where tolerated.
Prioritising sleep and stress management.
Participating in regular resistance training and physical activity.
Working with your GP, endocrinologist and accredited healthcare practitioner to develop an individualised management plan.
When Should You Speak With Your GP?
If you are experiencing persistent symptoms such as:
fatigue
brain fog
unexplained weight gain
hair thinning
heavy menstrual bleeding
elevated cholesterol
constipation
feeling unusually cold
it is worth discussing these with your GP.
Appropriate assessment may help determine whether thyroid function, nutritional status, perimenopause or another health condition could be contributing to your symptoms.

Perimenopause is a significant hormonal transition that affects many systems throughout the body.
For some women, thyroid dysfunction may also develop during this time, contributing to symptoms that are often mistaken for menopause alone.
Although "thyropause" is not an official medical diagnosis, it is a useful concept that reminds us to look at the whole picture rather than assuming there is a single explanation for every symptom.
